Lakeshore Lightning – Permission to Tryout (Permission to Skate)
Request Permission to tryout Form
If your player plans to attend tryouts with another OWHA association, you must request a Permission to Tryout (PTT) form (also called Permission to Skate).
What You Need to Do
1. Complete This Request Form - Request Permission to tryout
Fill out this form if you intend to request a Permission to Tryout.
2. Watch for Your Form
Permissions will be emailed after Provincials (April 10–12).
We will work to send them out as quickly as possible.
3. Review and Complete Carefully
Ensure all information is accurate and complete before submitting to the tryout association.
Providing full and correct details upfront will help avoid delays.
4. Bring to Tryouts
The completed and signed form must be provided to the association your player is trying out for before she is allowed on the ice.
Important Reminders
- This form allows your player to attend tryouts only. It is not a release.
- Players must have a valid Permission to Tryout or an official release to participate with another team.
- Failure to have a completed form may result in the player not being permitted to try out and could lead to further penalties.
