2025/26 Travel Coach Evaluation Form

We value your feedback and would appreciate you taking a few minutes to complete this short survey about your daughter’s and your family’s experience with Lakeshore Lightning and the travel team your daughter played on during the 2025–26 season. If you have more than one child playing travel hockey, please complete one survey per player. We also welcome input from all parents or guardians. Your input will help us understand what is working well and where we can continue to improve the travel hockey experience for our players and families.
