Instructions for Completing the Coach Evaluation From. Thank you for taking the time to provide feedback on the past season! The survey consists of 15 quick questions, where you'll rate your experience on a scale from 1 to 5 (5 = Great, 1 = Poor). The survey is divided into three sections: Your child’s experience , your experience as a parent and Feedback for the Board. Please enter your name when completing the survey.
